*DOR 825-28 Affidavit of Affixture - Department of Revenue
For a mobile home that is permanently affixed to real property of the owner.
*DOR 658 Affidavit of Removal of Affixed Mobile Home
*MVD 32-601 Non-Probate Affidavit - For obtaining title to the manufactured home from a deceased vehicle owner's estate.  
*MVD 48-9901 Lien Release - For a lien holder to release a lien on a manufactured home title.  
*MVD 960236 Title Application - Application to title and/or registration a manufactured home.
*MVD 48-1001 Power of Attorney - To appoint another person to sign all documents required to obtain and transfer a mobile home title.  
*MVD 38-4306 One in The Same Name Certificate - Supporting documentation when the name on an ownership document does not match the signature.  
*MVD 468502 Sold Notice - Must be submitted within 10 days of sale/transfer of manufactured home. This notice protects the seller from liability that occur after the sale. To transfer ownership, the transfer information on the title must also be completed.  
*MVD 46-0306 Release of Interest - For a customer to provide a notarized statement that he or she has no interest in the ownership of a vehicle being titled. 
*MVD 48-2004 Bill of Sale - Supporting documentation for a manufactured home title transaction.

ARIZONA TITLE REQUIREMENTS - NEW HOME PURCHASE

Title Fee $7.44 (as of 5/11/09) per section, payable to “AZ DMV” (title fee subject to change when postage changes)
**Additional $4.00 (per section/title/mso) penalty fee, payable to “AZ DMV”
**Title Fee Financed on Contract: $ DOC PREP: NO DUPLICATE TITLE FEE FINANCED OR CHARGED
Sales tax – if financed, make payable to dealer
**Proof Sales Tax has been paid (only if sold out of state) ORIGINAL ONLY - PAID RECEIPT
MSO – copy of if there is flooring
First/Second Assignment should be completed by retailer, can be completed by POA – must have dealer number
If no flooring, must have original MSO(s) in file
Legible copy of FINAL invoice

**Mobile Home W/L to be completed (take from MSO), VIN # (from MSO), Make, Body Style (MH), Year (last 2 numbers, i.e. “00” or “01”, First Registered (MSO), Model
Lien Amount – will always be the “Amount Financed” taken from the contract
Lien Date – Date contract was signed by borrower
Owner Info: Owner Driver License or SSN – required
**Owner Name, First, Middle, Last
**DOB – required
**Legal Status – if two applicants, then status will always be “OR”, owner will need to sign in section (can be signed by POA)
**Street Address, City, State, ZIP – should match the property locator. County is not required.
**Mailing address – if different from actual location
**2nd & 3rd Owner Section to be completed if more than one applicant, in the same way as for the first owner. Vehicle/Mobile Home Location – can read “same as above” UNLESS there is a different mailing address, then need to write/type in address from property locator
**Date Vehicle Acquired – date home sold
**Seller Name – should match what is on MSO or Purchase Agreement, write in their city and state
**Must be signed by all owners, can be signed by POA.
Power of Attorney for Buyer(s) (notarized) – 2 originals, either left blank or MUST match MSO or invoice, names must be spelled correctly
Power of Attorney for Dealer (2 originals – notarized), MUST match MSO (for all dealers listed)
Dealer’s Purchase & Sale Agreement – copy, must be signed by all purchasers and dealer’s representative
**Must have current date of sale or penalty fees for titling must be paid/financed
Payoff Authorization form, signed by Dealer Representative
**Balance information must be provided, if differs from invoice request explanation of difference
**Overnight address – no PO Boxes

Documents to send to DMV: Cover Letter, Title Fee Check, title application(s), MSO(s), POA if used to sign, purchase agreement

NO CROSSOUTS ALLOWED ON POA’S!!! MUST HAVE NEW FORMS IF THERE IS.
