Statement of Partnership or Joint Ownership - Used in conjunction with a registration/title application when more than two persons are partners/joint owners of a Mobile Home.

Registration Authorization - Authorization by owner for registration of a Mobile Home in another person's name (e.g. under a lease). This form not required when "If the Registrant is not the Owner" section on form MV-82 is completed. Individual owner's proof of ID and date of birth required when submitting either form.

Transfer of Mobile Home Registered in Name of Deceased Person - Use to transfer the Mobile Home of a deceased person when there is no will, estate, surviving spouse or surviving minor child.

Application for Duplicate Certificate of Title - Use this form to request a duplicate (replacement) Mobile Home title by mail. The duplicate title fee is $20. When using this form to request a duplicate title by mail, you must include a photocopy of your identification.

Notice of Repossession of Motor Mobile Home or Motorcycle - Use to notify DMV, as required by law, when a Mobile Home is repossessed. Three copies of the form are included. Present all three copies to a DMV office and the copies marked for the Repossessor and the Owner will be returned to you. See also form MV-950.

Notice of Lien - Filed by lien holder to record a lien on a Mobile Home title. A $5.00 fee must be paid by lien holder. Both lien holder and owner must sign form. If a title has already been issued, original title must be submitted with this form.

Notice of Lien - Charge Account Customer - Filed by a lien holder who has a charge account with the DMV to record a lien on a Mobile Home title. If you do not have a charge account, use form MV-900. A $5.00 is charged to the escrow account of the lien holder. Both lien holder and owner must sign form. If a title has already been issued, original title must be submitted with this form. |

Affirmation of Repossession and Bill of Sale - Required to be completed when a lender repossesses a Mobile Home, and used to transfer the Mobile Home to a new owner. |
