SUT 1	Supplemental Information for Procurement of Title - A Manufactured Home seller and the buyer use this form to serve as a formal bill of sale when a vehicle has been purchased or sold with a questionable sales price.
VSA 17B	Certificate of Title - Manufactured Home, Application for - Use this form to title a manufactured home.
VSA 66	Application for Lien - Use this form to apply for a supplemental lien, lien transfer, replacement title certificate or substitute title certificate.
VSA 70	Power of Attorney- Ownership of Manufactured Home - A vehicle owner uses this form to appoint an individual power of attorney to sign for the owner when registering and/or transferring ownership of a Manufactured Home.
SUT 4	Transfer of Certification of Lien Information - Use this form to record the transfer of a certification of lien information.
VSA 12	Affidavit in Lieu of Title Certificate - Use this form to file an affidavit with DMV certifying that the certificate of title is not in the applicant's possession when registering a Manufactured Home without a certificate of title.
